I hereby release Phillip Vitug, Conservatory of Dance Education or it's agents and employees from any all injuries my child may sustain while under the supervision of CODE and the instructor or employees are not liable for personal injuries, loss or damage to personal properties. You must inform the instructor of any personal limitations you may have for any activity in which you participate. If you have any doubt to your physical abilities, please consult your doctor before participating.
